It seems like my entire yard is full of weeds. How do I get rid of the weeds and what should I do to get grass growing and keep it going?

I have been using the Scott's Turf Builder fertilizers on the recommended schedule and had my lawn aerated last fall. I also have a sprinkler system. Not sure what type of grass I have, but I live in Annapolis.
